- Referring providers who would like an outside wound assessment

What to Expect
- Measurement of the wound's size and depth
- Evaluation of the wound bed, edges and surrounding skin
- Review of drainage, odor and any signs that may need additional attention
- Discussion of health history, mobility, nutrition and circulation as they relate to healing
- A written, individualized plan for next steps
When to Seek Care
- Any wound that has not been evaluated by a healthcare professional
- A wound that looks different than it did previously
- A wound you or a caregiver are unsure how to manage
